Submitting a Military or Government Claim

Fill out the claim form COMPLETELY, particularly boxes #8, #9 & #10

inventory #, purchase date/price & amounts repair/replace

Incomplete information may result in delays in handling of your claim. 

 ·        Please attach the following to your dated completed claim form: 

FOR HOUSEHOLD GOODS CLAIM: 

1.      Copies of original purchase receipts (if available)

2.      Repair Estimates if claiming over $100.00 each.  (Let us know if you need assistance in locating repair firms and you located in the United States including some areas of Hawaii and Alaska)

3.      Replacement Estimates to damage & missing items if over $100.00 each.

4.      Photographs of the damaged items.

5.      DOCUMENTS showing damaged/loss items were notified to the carrier during the time of delivery. 

6.      ****Military Claims only:  Please make sure that your DD 1840/1840R is filed within 70 days of delivery to the Military Claims Office OR 75 days to the carrier)

 

FOR VEHICLE (AUTO & MOTORCYCLE) CLAIM: 

1.      Two (2) Estimates of repair – repair estimate must state the damages the vehicle sustained and what type of repair or replacement will be done.  

2.      Photographs of the vehicle showing areas of damage

3.      Documents made at delivery regarding the damages sustained.

  

  • You have 75 days from date of delivery to notify the carrier IN WRITING of all items being claimed for loss or damage or it will be considered not to have occurred in transit. 
     
  • You have 2 years to file your claim to the Military or to the carrier from the date of delivery. 
     
  • Do not discard broken items being replaced.  The carrier has SALVAGE rights to all items replaced.
